I just tried a different page name other than index (tag-index) and that works fine, so something is intercepting the ../index page url and not finding it. Probably some default .htaccess or proxy setting.

Sounds like a permalink issue, which is usually caused by either your WordPress settings or hosting configuration.

I created a tag index page called index following the Index Page section of the theme help file, but the page fails to display either on the site, or when I'm in the page editor and it the preview button. Any idea what the issue is?
