What I added is
$lowmsg = strtolower($message);

I guess it might be related to which php version you are using. Maybe my version (PHP 5.4) is more strict on this.

Also, you might want to add instructions to the documentation to help user set up SPF for user's domain and change the "From" to the template user's email. Because otherwise, the email might be detected as spam, as it sets the "From" header to the visitor's email but the email isn't from the visitor's email server.

So.. you have "if((eregi($suspect, $lowmsg)) || (eregi($suspect, strtolower($_POST['name']))) || (eregi($suspect, strtolower($_POST['email']))))" on line 48 of send.php. But you don't have $lowmsg defined anywhere.

As the title, I think you just missed the $lowmsg = strtolower($message);
