With the way WordPress generates the category list, only categories with published posts will appear.

Regarding captcha, something like that would require a customization as it isn't a quick edit and can provide instructions for here on the forums. You can submit requests for that here: http://themolitor.com/customization-request

My form submission routine is working perfectly! But I only have one category in the event type...festivals....how do I create more...I do have a whole bunch in my category panel
